BOSENTAN INTERACTIONS WITH WARFARIN (Coumadin, Jantoven)

During the initial testing of bosentan, it was found that the activity of warfarin (Coumadin, Jantoven) was decreased when the two were given to healthy males. Since these types of tests may not be directly applicable to people with active disease it is important to look for reports and studies involving actual patients. The package insert for Tracleer that there was no difference if the changes made to warfarin (Coumadin, Jantoven) dose between people taking the drug and those treated with placebo during clinical trials.

There has been a report of a greatly decreased INR when bosentan was added to warfarin (Coumadin, Jantoven) therapy. This patient needed about 64% more warfarin (Coumadin, Jantoven) going from 27.5 mg/wk to 45 mg/wk.

Since there are conflicting reports and a potentially dangerous drop in warfarin (Coumadin, Jantoven) activity, it seems prudent to have the INR checked about one week after bosentan is started on someone who is already taking warfarin (Coumadin, Jantoven).
