PHENYTOIN INTERACTION WITH WARFARIN |

Brand name: Dilantin THIS IS A VERY DANGEROUS INTERACTION. BOTH INCREASES AND DECREASES IN THE INR HAVE OCCURRED. LOSS OF ANTICOAGULATION AS WELL AS LOSS OF SEIZURE CONTROL HAVE OCCURRED WHEN PHENYTOIN HAS INTERACTED WITH WARFARIN. YOU MUST HAVE CAREFUL AND FREQUENT MONITORING WHEN THE DOSES OF THESE MEDICATIONS ARE CHANGED. The interaction seems to be very strong both when phenytoin is added to warfarin and when it is stopped while while warfarin is continued. There is no substitute for frequent monitoring of the INR when any change is made. You must have the INR and the phenytoin level determined about every three days until both have stabilized.
