WARFARIN DOSING ALGORITHMS
This is a single sheet of 110 pound white card stock. It will be folded into thirds. Each section then contains one algorithm in flow sheet format. This is suitable for a clinician's lab-coat pocket, or a doctor's office. It can also be used by those who test their INR at home and adjust their own warfarin doses. This book is useful for those who are learning to manage anticoagulation therapy. It is best done with two people (each with their own copy of the book). Because of this, if two copies are ordered at the same time and shipped to the same address, the second copy will be at half-price. Each case study illustrates a principle of anticoagulation management. Many of the cases require a recalculation of the warfarin dose. To assist in dosage changes, the algorithms mentioned above are included. The only difference is that they are not printed on card stock.
