TRANEXAMIC ACID MOUTHWASH FOR ORAL BLEEDING

PLEASE NOTE:  THIS PRODUCT IS NOT COMMERCIALLY AVAILABLE IN THE UNITED STATES.  IT HAS NOT BEEN APPROVED BY THE FOOD AND DRUG ADMINISTRATION.

This product has been shown to be effective in stopping bleeding after dental procedures and biopsies in the mouth.  Since it is not a ready-made product, you will not be able to get it from most of the big pharmacy chain stores.  They depend on filling large numbers of prescriptions and usually are not interested in taking the time to make this product.  In addition your insurance company may not pay for it unless it is approved prior to billing.  The best way to get this will be to contact one of the small, independent pharmacies in your area.  They are more likely to know how to get the ingredients, know how to combine the ingredients and take the time to make a one-time prescription.  You can find one of these businesses listed under pharmacies in the Yellow Pages, look especially for one that lists compounding as a specialty.

The following is an e-mail exchange that I had with a visitor to this site.

I am on warfarin because I have a mechanical heart valve.  My oral surgeon did a biopsy on an area in the roof of my mouth when my INR was 2.2.  It took quite a while for the bleeding to slow down.  It has not stopped because I keep bumping the area when chewing food and rubbing it with my tongue.  Do you know anything that we could use to stop the bleeding?

There is a chemical called tranexamic acid that can be made into a mouthwash containing 4.8% of the active ingredient.  (I gave him the information about it being difficult to get that is at the top of the page.)

I found a pharmacy that knows how to make it but there is none of the tranexamic acid powder available in the Kansas City area.  It comes from Houston and usually takes three days to arrive. 

Since this was the Friday before a three-day holiday weekend, this did not look very promising for immediate relief.  On the Tuesday after the holiday I received another e-mail.

They had the powder shipped overnight from Houston.  The oral surgeon ordered 300 ml of a 4.8% solution (this would require 14.4 g, about 1/2 ounce of the powder.)  I am to rinse the area 4 times daily for one week.  The bleeding stopped immediately after the first time I used it.  After 4 days, I have not have had even a streak of blood any time I rinsed or spit. 

If any reader of this page has had any experience with tranexamic acid mouthwash, I would like to hear from you.  If you send your experience and give me permission, I would like to add it to this page.

I will also start a list of pharmacies that can make this product if you would like to send in that information.
