TRAZODONE INTERACTIONS WITH WARFARIN

  An article by Small and Giamonna cites three case reports in which trazodone appears to have interacted with warfarin. When the patients were taking warfarin and started taking trazodone, the INR decreased and the dose of warfarin had to be increased.  When the patients were taking both trazodone and warfarin and then stopped the trazodone, the INR increased and the dose of warfarin had to be decreased.  Exactly how this occurs is not known.

  The authors conclude, "The use of trazodone on an as-needed basis for sleep is strongly discouraged in patients who are receiving warfarin..."
